You can apply for CHIP or Children's Medicaid by downloading, printing, and mailing a copy of the CHIP/Children's Medicaid application or by calling 1-877-KIDS-NOW (1-877-543-7669) for an application, instructions on how to fill it out, and a postage-paid return envelope. To apply, you will need proof of your family's current income, proof of U.S. citizenship or immigration status for all children applying for coverage, and proof of expenses for child care, disabled adult care, child support and/or alimony.
